Form A - Application for Accreditation of a Corporation as an Animal Research Establishment

To be completed by corporations applying for accreditation as Animal Research Establishments. See Fact Sheet 2: Applying for accreditation as an Animal Research Establishment.

Form C - Animal Research Authority

This form is for corporations issuing Animal Research Authorities to investigators and teachers. Animal Research Authorities are issued, subject to Animal Ethics Committee approval, specific to each research/teaching project. See Fact Sheet 5: Animal Research Authorities.

Form D - Application for accreditation of a corporation as an Animal Research Establishment (SCHOOLS)

To be used by schools applying for accreditation as Animal Research Establishments that intend to nominate supervision by the Schools Animal Ethics Committee. Form D (2) - Application for accreditation (SCHOOLS)

Form to be used by schools applying for accreditation as Animal Research Establishments that intend to establish their own Animal Ethics Committee. Form J - Application for an Animal Supplier's Licence

Form to be used by corporations or individuals applying for an Animal Supplier’s Licence (i.e. those  intending to obtain, breed or supply non-exempt animals for research purposes). See Fact Sheet 6: Application – Animal Supplier’s Licence.

Form L - Animal Use Statistics

Form to be completed for animal use in research and teaching on a yearly basis from 1 January to 31 December. Submit the completed form to NSW DPI by 31 March of the following year.

Form N - Licensed Animal Supplier Record for Dogs and Cats

This form is to be used by Licensed Animal Suppliers when obtaining, breeding or supplying dogs and cats for research purposes. See Fact Sheet 21: Supply of dogs and cats for use in research.

Form P - Application for Ministerial Concurrence to Perform LD50 Tests

To be completed by corporations and individuals wishing to carry out LD50 tests. See Fact Sheet 10: Draize tests, LD50 tests and lethality tests requiring death as an endpoint. 

Form Q - Declaration for Release of an Animal for Research 

Form for waiver of Animal Supply requirements for animals with specific medical or genetic conditions.
