Form C - animal research authority This form is for corporations issuing Animal Research Authorities to investigators and teachers. Form L - Animal Use Statistics Form to be completed for animal use in research and teaching on a yearly basis from 1 January to 31 December. Submit the completed form to NSW DPI by 31 March of the following year.	338.5 KB
